Case Manager
United Community Family Services looking for a Case Manager to join our team. You will be supporting the agency through various programs and services.
JOB DESCRIPTION
- Responsible for multi grant programs with wrap around services.
- Documents all contacts and services in client case files and maintains case note logs.
- Tracks client progress and provides follow-up assistance throughout service delivery.
- Acquires, retains and coordinates with enrolled clients for programs and services.
- Provides administrative assistance and coordination to support programs and agency as needed
- Using various software, including Microsoft office, salesforce, MiBridges, and other internal agency systems.
- Strong organizational and time management and case management follow-up skills are required.
QUALIFICATIONS
- Bachelor’s degree preferred
- Candidate must be bilingual (English/Arabic) with strong interpersonal, written and oral communication skills, as well as proficient in computer skills.
- Ability to effectively interact with individuals from diverse cultures and ethnicities
- Ability to work well independently and as part of a team
- This is a full-time, non-exempt position.
Job Type: Full-time
Pay: $50,000.00 - $52,000.00 per year
