Community Care RN
Status: Exempt, min 40 hours/week
Location: Ypsilanti, Huron Valley PACE
Overview
The Community Care Registered Nurse works in a close collaborative relationship with a Nurse Practitioner and Licensed Social Worker to provide integrative care to frail elders with complex needs.
What you’ll do
- Provides comprehensive care to participants
- Assesses participant needs and plans for appropriate care
- Collaborates with the participant, family, and Interdisciplinary Team to develop the participant’s plan of care
- Teaches, supervises and counsels the participant, caregiver, or family regarding care needs and related problems utilizing adult learning principles
- Initiates preventative and rehabilitative procedures or programs as appropriate
- Monitors participant progress and notifies the appropriate personnel of improvement or changes in participant status
- Provides on-going coordination with home health services
- Evaluates participant outcomes and progress toward achieving goals of care plan
- Collaborates with the Interdisciplinary Team to revise the participant plan of care based on changes in physical and psychosocial status and initiates interventions consistent with the changes in status
- Collaborates with the participant, family, and members of the Interdisciplinary Team to evaluate/measure the individual and group response to care and teaching interventions
- Maintains accurate and timely records and documentation.
- Participates in the collection and documentation of Huron Valley PACE quality data
- Advocates to others on behalf of the participant
- Works toward achieving high levels of participant satisfaction
- Provides after hours on-call assistance via phone triage on a rotating basis
- Participates in mandatory training and screening including but not limited to: infection control, safety training, and BLS training
- Participates in and/or facilitates Quality Improvement projects resulting from data results
- Reviews current periodical literature relevant to general participant care and the PACE model of care
What it takes
- Registered Nurse (RN) with current Michigan licensure, BSN preferred
- Must possess a current State of Michigan driver’s license and maintain an acceptable driving record
- CPR Certification
