Unity House is opening a brand new six-bed Crisis Residence that offers a safe place for the stabilization of psychiatric symptoms and a range of person centered, trauma informed services to support participants.
This is a short-term stay facility that serves residents for up to 28 days as they prepare to transition back to the community.
This position will be available October 21, 2024.
Responsibilities
Conduct mental and physical health assessments.
Help create individualized participate service plans.
Participate in care coordination.
Work as part of a multi-disciplinary team to meet resident needs.
Provide linkages to relevant medical services and preventative care.
Promote overall wellness and self-management.
Provide accompaniment, support and advocacy as needed.
Participate in rotating on-call system.
Promote a culture of inclusion and belonging.
Requirements
Current, unrestricted NYS Licensed Registered Nurse. Must be current with CPR and First Aid certification. At least one year experience working in mental health/substance abuse settings. Clean and valid NYS driver license. Must be able to work independently and as a team. Proficient with computers
