What are the responsibilities and job description for the Housing Case Manager position at Unity Parenting and Counseling?
Primary Work Function:
Provide intensive case management services to homeless individuals and families in the program. Establish and cultivate relationships with community partners, ie. employment service agencies, child care facilities, parenting programs, realtors and/or landlords. Must be able to demonstrate sensitivity to the service population's cultural and socioeconomic characteristics.
ESSENTIAL FUNCTIONS
1. Meet with assigned clients on weekly basis or as needed (on site and/or scatter site locations) to provide developmentally appropriate services to help households overcome barriers to acquiring and maintaining permanent housing.
2. Recruit landlords and help households find appropriate rental housing in the community.
3. Develop and implement a service plan for each head of household with goals and objectives geared toward maximum independence.
4. Electronically update client information utilizing HMIS data system.
5. Provide or directly refer participants with needed services to include: psychiatric evaluation/treatment, mental health /direct services, substance abuse evaluation/treatment, inventory of competencies, skills inventory, and employment, counseling/placement, medical services, educational/job training resources, vocational training, public legal services and housing resources and referral.
6. Provide follow-up (after-care) services for six-months after program discharge.
7. Participate with other staff members in regular staffings held to determine service needs, progress toward meeting goals and appropriate staff intervention.
8. Complete all required documentation and maintain client files in order at all times. Document services within 48 hours of interventions.
9. Provide crisis intervention and link family members to necessary internal and external resources.
10. Actively participate in the agency CQI Plan.
11. Conduct apartment checks for cleanliness and repairs that may be needed and submit the repair needs to designated management staff.
12. Teach life skills classes, to include: budgeting (money management), housekeeping, communication, conflict resolution, good health, legal issues, transportation, community resources, pregnancy, prevention, housing options and employment and vocational services.
13. Implement a range of activity groups for adults and children, to include: socialization, cultural, and recreational activity.
14. Coordinate and link to educational resources for head of household youth and their children.
Training And Skills
Two years working with homeless families and two years working with chemically dependent individuals.
CADC Preferred.

Work Requirements:
Pass a CANTS (Child Abuse and Neglect Tracking System) check.
Pass a Criminal Background check.
Possess and maintain a valid Drivers License and possess safe and reliable transportation.
Possess and maintain adequate Liability Insurance for the vehicle.
