POSITION SUMMARY/RESPONSIBILITIES
In collaboration with case managers, physicians, staff, community health resources, and payers, is accountable for assisting social workers and case managers with coordination of care secondary to patient/family psychosocial/socioeconomic needs across the care continuum. Ensures timely care, compliance with regulatory requirements/standards, and effective/efficient use of hospital resources. Completes established competencies for the position within the designated introductory period.
EDUCATION/EXPERIENCE
High school diploma required. Associate degree preferred. Six (6) months of experience in a healthcare environment are required. Preferred experience in HMO, hospital, home health or any clinical setting involved in discharge planning
