Job Summary
Under the direction of the Director Women’s and Teen Center (WTC), helps coordinate patient care, connects patients with educational and financial resources, and helps patients understand the healthcare system.
Assists with the non-clinical aspects of patient navigation through the healthcare system to result in timely care. Duties are performed at different sites.

Work Experience
Two years of customer service experience required. Experience as an Educator in Maternal-Child Health or community education highly preferred.
Experience working with MS Windows, Word, and desktop publishing required.
License / Registration / Certification
Current CPR training required.
Current State valid driver’s license required.
Liability insurance required.
Certification Health Education Specialist (CHES) preferred.
Education and Training
High school diploma or equivalent. Bachelor degree in Health Sciences, Education, Community Health preferred.
