Responsibilities
Job Summary
The Medical Records Coder is responsible for working across the professional fee organization; responsibilities include but are not limited to system edit review associated with clean claim submission collaborating with accounts receivable and other stakeholders. Assigns codes (ICD-diagnosis, CPT, HCPCS and modifiers) from appropriate coding classification system to assure the production of quality healthcare data and accurate professional payment. The coding function ensures compliance with established coding guidelines, third party reimbursement policies, and Federal and State regulations.
Key Functions and Expected Performances
With general direction of the Manager of Coding:
30% Uses thorough knowledge of coding systems and system logic to review codes created by electronic charge capture and/or assign codes (ICD-10-CM, E/M, CPT, HCPCS and modifiers) through medical record documentation as per designated workflow. Processes and posts electronic remittance advice.
20% Ensures work queue and in-basket responsibilities are handled within established guidelines and timeframes. Troubleshoots any problems that prevents claims from being released. Identifies cause of edit and independently resolves issue by reviewing the patient encounter to understand the nature of the problem.
20% Consults with charging provider or department to obtain greater specificity and/or clarification. Queries providers to clarify conflicting, imprecise, incomplete, ambiguous, and/or inconsistent clinical information when appropriate. Inquires. Communicates documentation improvement opportunities and coding issues to appropriate personnel for follow up and resolution.
15% Prepares reports for management to document recurring problems and identifies the source of reimbursement delays. Works closely with management to ensure effective communication to resolve invoice payment delays. As necessary, provides Providers and other staff with information relative to coding.
May perform other duties as assigned.
Qualifications
High School Diploma.
1 yr experience in a healthcare billing or coding position
or equivalent combination of education and experience.
Abides by Standards of ethical coding as set forth by American Academy of Professional Coders (AAPC} and American Health Information Management Association (AHIMA) and adheres to official coding guidelines.
Maintains coding credential requirements.
Maintains required productivity and quality requirements.
Ability to deal professionally and courteously, and efficiently with the colleagues and providers and to remain calm under stress.
Ability to understand and interpret policies and regulations.
Ability to examine documents for accuracy and completeness.
Ability to communicate effectively.
Certification in one of the following: Certified Professional Coder (CPC, CPC-A) examination through American Academy of Professional Coders, American Health Information Management Association (AHIMA) accreditation examination for Registered Health Information Administrator (RHIA) or (Registered Health Information Technician) RHIT or Certified Coding Specialist (CCS, CCS-P) or Certified Coding Associate (CCA®) preferred
