Position Summary:
Dedicated social worker assigned to the emergency room to work high risk patients and patients that frequently utilize the emergency department, with the goal of providing community resources and health care navigation-with the ultimate goal of reducing ED visits. This position also entails some discharge planning for emergency room patients to assist in reducing overall throughput.
Position Qualification:
Social Work degree
