Job Summary
The Utah Lions Eye Bank, located in Murray, Ut, is a nonprofit organization and has facilitated the gift of sight worldwide through transplantation and research since 1972. By restoring vision, cornea donation changes not only the lives of recipients, but also the lives of everyone around them. We offer a generous benefits packaging including medical, dental, and employer paid retirement. This position also includes shift differential compensation and is eligible for half-time remote work. The Eye Bank Donor Coordinator is responsible for processing all donor referrals by telephone and e-mail; perform medical screening to determine eligibility and suitable of referrals for eye donation; obtaining legal authorization for eye tissue donation; coordinating the recovery of eye tissue by Eye Bank Tissue Recovery Specialists; obtaining a medical/social history from donor family; communicating effectively with all referral sources; initiating, maintaining, and completing donor records; maintaining referral/donor information in the Utah Lions Eye Bank database. Performing medical review on completed cases.
Responsibilities
The Eye Bank Donor Coordinator will be responsible for the following:
- Document all information for each potential donor referral in the ULEB referral database.
- Interview referring hospital healthcare professionals concerning medical history of potential donors to determine donor eligibility.
- Screening every potential case in relevant Electronic Medical Records ( EMR ).Perform preliminary plasma dilution assessment of potential donors with information obtained from the healthcare professional and/or code sheets, EMS reports, and transfusion records to determine whether or not a pre-transfusion blood sample will need to be obtained by the Eye Bank Tissue Recovery Specialist.
- Contact Authorizing Decision Maker of potential eye donors over the telephone to present the option of eye donation and answer any questions they may have about this process.
- If the family chooses to donate, the Donor Coordinator will obtain recorded authorization over the phone, and complete a medical/social history questionnaire over the phone.
- Coordinate ocular tissue recoveries for each donor with the on-call Eye Bank Tissue Recovery Specialist and the transportation of ocular tissue procured from outlying locations to the ULEB office.
- Print copy of donors medical chart prior to recovery and upload into database, or arrange for a copy of the hospital chart to be made and provide the location of the chart to the Eye Bank Tissue Recovery specialist if unavailable in the EMR .
Applicants expected to train and work in person. The option of up to 50% remote work is available after successful completion of the probationary period.

Minimum Qualifications
Associates degree in sociology, psychology, social sciences, a related field or equivalency (one year of education can be substituted for two years of related work experience); plus one year of related experience. Demonstrated human relations and effective communication skills also required. Certification as an Eye Bank Technician or Nursing Assistant is preferred
