Duties and Responsibilities:
The Registered Nurse (RN) in Home Health provides high-quality patient care in their homes, as prescribed by physicians and in accordance with applicable laws, regulations, and agency policies.
You will coordinate total care plans with other healthcare professionals, ensuring continuity of patient care through effective communication and collaboration.
Your responsibilities include:
- Making initial and comprehensive nursing evaluation visits
- Developing personalized care plans within state-specific guidelines
- Submitting accurate, complete, and timely documentation in collaboration with physician orders
- Performing patient assessments at designated time points and developing appropriate care plans
- Ensuring patients meet home health eligibility and medical necessity guidelines
Licensure Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
