The Program Specialist will be responsible for the timely completion of assessments paperwork, which includes but is not limited to annual assessments, monthly and quarterly reports. Participating in client’s annual Individual Support Plan meetings. Will be used as coverage to remain in compliance if short staffed.
The incumbent will be joining a dedicated, supportive team. The ideal candidate will be a caring team player who has knowledge of individuals with intellectual disabilities, strong time-management and organizational skills, and the ability to stay calm under pressure. This position works Monday through Friday, 8:00 AM to 4:00 PM.
Responsibilities:
- Plan Development & Implementation: Accurately develop and implement Individual Support Plans (ISP), Fade Plans, SEEP, and Behavior Plans in compliance with Allegheny County and Department of Public Welfare regulations.
- Assessment & Documentation: Conduct assessments for up to 45 individuals, coordinate reconciliation meetings, and ensure thorough documentation of ISP reviews and updates.
- Team Coordination: Provide documentation to team members, inform them of their rights, and instruct direct care staff on health and safety needs.
- Service Monitoring: Monitor and evaluate services provided to participants, report discrepancies, and act as Plan Lead when needed.
- Direct Support: Offer direct care, personal assistance, transportation, and support during evenings as needed.
- Training & Compliance: Complete 24 hours of approved annual training and ensure adherence to company, county, and state policies.
- Vocational Support: Write, implement, and monitor vocational goals; prepare payroll sheets for submission.
- Quality Assurance: Conduct quarterly chart checks and review monthly documentation of individual progress.
- Behavior Management: Develop behavior management plans for individuals without an authorized Behavior Specialist.
- Team Relations: Maintain effective, cooperative relationships with consumers and team members to foster a positive work environment.
