What are the responsibilities and job description for the Service Coordinator Lead (Safe Harbor Behavioral Health) position at UPMC?
The Service Coordinator Lead will assist Supervisors in maintaining overall responsibility for day-to-day program operations and development. In cooperation with the supervisor, ensures that Service Coordinators understand and incorporate standards and regulations into their daily activities. Additional responsibilities include maintaining system-wide competencies such as 30, 60, 90 day check-ins with staff, suicide screenings and cultural competencies.
Our Safe Harbor team has a unique and innovative program with the Erie School District that allows us partner with 16 diverse schools. Our team serves both student and families to reduce chronic absenteeism and truancy as well as address other non-academic barriers. Service Coordinators work within the Community School model, collaborating with school Administration, Community School Directors to meet the needs of our students and families. We partner closely with external service providers and are heavily engaged within the community.
This position will have Performance Standards that will be reviewed yearly. The dress code is business casual. The schedule for this role is Monday through Friday, mostly daylight hours, 830a-5p, with occasional flexibility in schedule required to support team needs.
Responsibilities:
- Adhere to the practices as outlined in the Service Coordination On Call guidelines. Assist service coordinators and the individuals receiving services in advocating for desired services both within the clinical treatment setting and within the community. Attend agency and state trainings as appropriate. Identify training needs and ensure presentation of appropriate training as necessary. Completes compliance and quality audits ensuring regulatory and best practice standards are met. Provides feedback and coaching toward improvement. Participates in the development and implementation of program policies and procedures. Evaluate all service coordination staff on a regular basis in collaboration with Supervisor/Program Coordinator/Program Director.
- Maintain linkages with the mental health community and other relevant health and human services. Assist the supervisor with the management of day-to-day program operations and development. As directed by the supervisor, provide group supervision and individual supervision as needed. Monitor the on-call rotation schedule for service coordinators. Participate in recruiting, hiring and training of service coordinators.
- Review service coordinators? documentation, including assessments, service plans, progress reports, correspondence and other related monitoring and reporting documentation. Review referrals for service coordination, assigns and monitors caseloads. Assists the supervisor with the management of day-to-day program operations and development Completes Compliance and Quality audits ensuring regulatory and Best Practice standards are met. Provides feedback and coaching toward improvement Reviews referrals and assigns and monitors caseloads Co- supervises up to 9 SC?s, both in office and during field supervision with completion of written documentation of supervision Develops meeting agendas and leads team meetings Participates in the development of department policies and procedures Handles complaints and investigations in coordination and under the direction of the supervisor Participates in recruiting, hiring, and training of Service Coordinators
