Responsibilities
This position is in radiology, based at the Strada Patient Care Center and will provide individualized assistance to patients, families, and caregivers to achieve patient-centered, evidence-based care; acts as a link between patients and care providers to facilitate radiological services and empowers patients to be an active part of the care team; works with patients to overcome obstacles to disease management by utilizing all available resources, both internal and external, to improve care outcomes; serves as Patient Navigation to USA Health Physician Group radiology services by providing information, support, encouragement and assistance to patients and family caregivers to overcome barriers in receiving care; stays abreast of all aspects of service line care and treatment methods; maintains navigation records in a timely manner using a database; maintains confidentiality and exhibits professional conduct when interacting with patients, family caregivers and medical professionals; Organize patient schedules and manage appointments to facilitate timely services; Facilitates scheduling of radiologic appointments; Works with radiologists and physicians with teleradiology including uploading images in PACS and sending out images to outside care providers. Participates in service line performance improvement activities; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.
Qualifications
High school diploma or equivalent and two years of related experience; or successful completion of a vocational program in a healthcare related field and one year of related experience. Higher education may substitute on a year-for-year basis for the required experience.
