POSITION SUMMARY
The Home Health Scheduler is responsible for creating and coordinating the home health schedules for assigned clinical teams scheduling patients for multiple home health services (Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Worker, Phlebotomist, and HHA). This position requires a variety of skills to maintain home health visit schedules and is responsible for training and supporting other employees in the scheduling positions/process. Facilitates, schedules, and manages interactive scheduling meetings with clinicians on a regular basis in collaboration with the therapy and nursing supervisors. Acts as the first point of contact for patients who call the home health agency, providing customer service and expectation management before routing the call appropriately. This position requires knowledge and competency with use of computerized scheduling systems.
ESSENTIAL JOB KNOWLEDGE
- Answer incoming phone calls and make outbound phone calls, gathering information to route appropriately.
- Schedule appointments using information received via phone and/or electronic messaging.
- Field incoming messages from patients and team members.
- Manage information in EHR In-baskets.
- Maintain clinical team daily work schedules/templates.
- Assist and act as liaison with Admissions in planning and scheduling patient visits.
- Contact patient prior to start of care to set up initial appointment.
- Serve as resource person and provide information to clinicians regarding Medicare regulations to
- ensure appropriate visit types are scheduled.
- Schedule visits with Interpreter Services. Confirm date and time with patient.
- Manages information in EHR in-basket as assigned.
- Coordinates communication with referral sources including NTUC, Episode creation and SOC calls as assigned.
- Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
- Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
- Quality: Focuses on improving processes and champions efficiency in daily work activities.
- Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).

JOB REQUIREMENTS
Licenses:
- State of Wisconsin Driver’s License
Education:
- High school diploma or equivalent required.
- Associate or bachelor’s degree in business or healthcare administration preferred.
Experience:
- Minimum of one year of recent experience working in customer service.
- Experience and strong knowledge base in healthcare or home health preferred.

Salary : $34,300 - $43,400
