Job Summary:
The position serves as an intake assessment nurse and will support in other nursing functions as needed.
Duties/Responsibilities:
- Performs assessments to determine medical eligibility for program.
- Educates qualified leads as to what can be expected within the PACE program.
- Coordinates intake activities with other intake team members including medical and financial eligibility and enrollment activities.
- Reports qualified leads to the team for safety in the community review.
- Provides back-up nursing assistance as needed and directed by DON.
- Participates in Interdisciplinary team meeting, safety in the community the community review, care planning as needed, and other meetings and committees as directed by the DON and/or Program Director.
- Utilizes the Health Information Exchange to gather information about candidates to complete intake assessments.
- Assists with developing annually departmental policies and procedures.
- Tracks and reports mileage and other expenses using the HR software in a timely manner.
- Other duties as assigned.
