Hospice Community Liaison – Veteran & SDOH-Focused | Valley Care Hospice
Locations: East Valley- Mesa, Tempe, Chandler, Apache Junction, Gilbert, Queen Creek, Gold Canyon, San Tan, Queen Creek
Schedule: Monday–Friday (Flexible availability as needed)
Job Type: Full-Time
Starting Pay: $75,000-$85,000 is your base pay; Depending on base admissions during probationary period uncapped bonuses above your base admission requirements| Post-probation base pay increase: based on performance during probationary period. Our marketers average between $95,000-$120,000 with their bonuses. This position is right for experienced healthcare liaison that's been successful looking for new change in making an impact on veterans healthcare.
About the Role
Valley Care Hospice is seeking a high-performing Hospice Community Liaison with a proven track record and an active, loyal book of business. This is more than just a sales role—this is a mission-driven opportunity to bring compassionate, expert-level hospice care to veterans and underserved populations (SDOH) across Arizona.

What You'll Do
- Cultivate and maintain strong relationships with physicians, hospitals, care facilities, and community partners.
- Actively market hospice services tailored to the unique needs of veterans and underserved populations.
- Lead outreach efforts with integrity, transparency, and ambition—backed by performance, not excuses.
- Partner with our interdisciplinary team to ensure continuity of care and positive patient experiences.
- Educate referral sources and the community on our unique value-based care model and veteran-focused expertise.
- Represent Valley Care Hospice at community events and professional engagements.
- Identify strategic growth opportunities and offer creative solutions to expand our impact.

Requirements
- 3 years of experience in healthcare marketing, business development, or referral sales.
- Demonstrated success in referral development and territory growth.
- Existing, active referral relationships in the region.
- Working knowledge of hospice care, especially for veterans and SDOH populations.
- Reliable transportation, current auto insurance, and a valid driver’s license.
- VA experience or knowledge strongly preferred.
