What You’ll Do:
As a Re-entry Transition Care Manager at Valley Oaks Health, you will have a dynamic and rewarding role that is essential to our core values and mission. Your day-to-day responsibilities will include:
- Collaborate with hospital discharge planners, correctional facility staff and community resources to develop individualized transition plans that address housing, medical care and mental health services for individuals re-entering the community.
- Help clients apply learned skills to various environments, including work, school, and home, ensuring skills are practical and sustainable beyond training sessions
- Build meaningful relationships with clients to achieve outstanding results.
What You Bring to the Table:
We’re looking for a driven, adaptable individual with a passion for mental health, who thrives in a fast-paced environment and loves working with others to achieve big things. Here’s what we’d love to see in your background:
- Education: Bachelor's Degree or 160 credit hours, Associates Degree or 60 credit hours. High school diploma or GED with one year of relevant work, volunteer or lived experience
- Experience 1 year of relevant work, volunteer or lived experience
- Required Valid Drivers License

Job Type: Full-time
Pay: $16.09 - $20.99 per hour
Expected hours: 40 per week
