Summary
- Oversee nutritional well-being of our residents by conducting nutrition assessments, identifying patients at risk, and creating diet plans.
- Collaborate with Dietician, physicians, therapists, and nurses to tailor dietary services and improve resident outcomes.
Essential Duties and Responsibilities
- Provide dietary services in accordance with company guidelines, state and federal laws and regulations
- Develop nutrition plans and implement interventions based on knowledge of patient's current health
- Complete monthly, quarterly, and yearly assessments of residents and adapt nutrition plans as needed Implement personalized, age-specific and culturally appropriate nutrition strategies
- Deliver client nutrition recommendations to physicians, nurses and aides
- Provide nutrition education and counseling for patients
- Any and all other duties as assigned
Working Conditions/Exposure
- Works and moves in office area(s) as well as throughout the facility
- Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances
- Is subject to frequent interruptions
- Works beyond normal working hours, and in other positions temporarily when necessary
- Is subject to a high exposure to infectious waste, diseases, conditions, etc., including TB, AIDS, and Hepatitis B viruses.
- Every precaution must be taken by using PPE and other safety devices.

Salary : $41,800 - $52,900
