What are the responsibilities and job description for the Family Nurse Practitioner/Physician Assistant position at Visiting Docs?

We are looking for a motivated, hardworking, and experienced Nurse Practitioner or Physician Assistant to join the growing Visiting Docs team.
The ideal candidate will have previous experience working with patients in their residences or in assisted living facilities.
Responsibilities include but are not limited to:
- Providing IN-PERSON general primary care services to patients.
- Diagnosing, treating and managing both acute and chronic conditions.
- Prescribing and refilling medications as appropriate.
- Actively communicating with patients, families, other specialists and facility personnel (case managers, social workers, med room, etc).
- Ordering and interpreting diagnostic tests such as imaging and lab work.
- Documenting patients’ histories and treatments in EMR.
Qualifications:
- Active unrestricted New York state Nurse Practitioner or Physician Assistant license.
- Minimum three (3) years of experience preferred.
- PECOS enrolled.
- DEA certified.
- BCLS.
- Knowledge of eClinicalworks preferred.
No evenings or weekends required. Ideally schedule would be Monday-Friday 9:00 AM - 2:00 PM.
