Overview
Responsible to assist families in identifying and accessing resources in coordination with the County Council for Young Children
Responsibilities
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
- Conducts CCYC-based outreach activities for community members including residents and stakeholders
- Establishes a network of trust within the community’s residents in order to offer needed community case management and resources
- Acquires and compiles intake information from residents to assist CCYC Manager
- Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications
- Associate’s/Bachelor’s degree preferred or a combination of education and experience
- 1-3 years of community health experience preferred; experience facilitating and conducting community education events is helpful
- Bilingual; Spanish preferred
- Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population
- Excellent verbal and written communication skills
- Strong organizational and interpersonal skills
- Valid and current driver’s license
