What are the responsibilities and job description for the Nurse Home Visitor position at Visiting Nurse Association of Central Jersey?
Overview
Responsible for providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program in Ocean County.
Responsibilities
Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
Attends weekly team meetings, case conferences and clinical supervision meetings
Assesses and evaluates needs of women and their families as they relate to health and life course development
Carries a caseload of up to 25 clients and completes home visitation in accordance with the NFP model elements and NFP Visit-to-Visit guidelines
Provides counseling and instruction to assist women and their families to attain targeted goals
Develops and maintains community relationships to support client referrals
Precepts and mentors new hires and makes visits with student nurses as needed
Adheres to agency infection control and safety policies
Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications
Bachelor’s degree in Nursing
2-3 years work related experience
Strong computer skills
Excellent verbal and written communication skills
Excellent interpersonal and organizational skills
Bilingual- Spanish speaking a plus
