We are currently looking for an hourly per diem Registered Dietitian Nutritionist for Ocean & Burlington County to join our team.  This position would be responsible for providing nutrition assessment and counseling to home care patients and their families.

Position Summary:

Responsible for completing comprehensive nutritional evaluations on all identified patients as per VNAHG policy

Meets all identified patients in their residence in the community to complete assessments or via Telehealth or by phone as per patient request.

Recent high risk pediatric experience a plus

Responsibilities

Principal Responsibilities:

Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers

Delivers a high level of customer service to assure positive patient experience

Completes comprehensive nutritional evaluations on all identified patients per VNAHG policy

Communication is maintained among team members as well as patient and care givers to ensure ability to implement nutrition plan of care

Adheres to agency infection control and safety policies

Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications

Job Competencies & Minimum Qualifications:

BS in Dietetics, Food and Nutrition

Registered Dietitian with the Commission on Dietetic Registration (CDR)

Experience with culturally diverse population

Strong communication, customer service and interpersonal skills

Computer experience preferred

Valid driver’s license

 

Working Conditions/Physical Demand:

Services are provided in the patient’s home in the community or facility setting or as per patient request via telehealth or phone.
