- Oversee the primary caseload of Home Health patients and provide skilled professional nursing care to them as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, PA licensure laws, and agency policies and procedures.
- Provide the comprehensive nursing assessment of patient needs and collaborate to develop the patient plan of care.
- Implement the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive, and responsive goals and interventions.
- Work closely with other members of the team to ensure all patient care needs are being met.
Qualifications:
- Graduate of an accredited registered nursing program.
- Bachelor’s degree in nursing preferred.
- Has minimum of one-year general nursing experience with current home health, medical, surgical, or critical care experience preferred.
- Ability to work within a Home Health setting.
- Registered Nurse licensed in the Commonwealth of Pennsylvania.
- Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.
- Cardio Pulmonary Resuscitation (CPR) certified.
