Key Accountabilities
Delivery of Scheduled Activities
- Plans and schedules daily activities for Memory Care residents.
- Assists Memory Care Manager with Activity Plan of Care for each resident.
- Transports residents from their room to activity location.
- Assists in leading activity groups, including exercise classes.
- Executes planned activities and social programming that is reflective of the residents’ interests.
- Follows direction to meet requirements of state, federal, and other regulatory entities as they relate to activities.
Promote Engagement among Residents
- Ability to learn and assist in other areas of the community.
- Assists residents with developing interpersonal relationships, socializing effectively, and developing the confidence needed to participate in group activities.
- Assists other departments with special events and family functions.
- Promotes a positive image of the community internally and externally.
- Promotes independence with residents by offering choices and fostering self-help skills.
- Reports any resident changes to the Nursing staff on duty.
Requirements
- Under the direction of Memory Care Manger, continuously assesses resident participation and engagement in activities and reports findings.
- Engages, encourages, motivates and escorts residents to planned activities.
- Implements programs that include spiritual, educational, intellectual, sensory motor, cognitive, social, health/wellness arts and crafts, music, exercise, games, and lectures.
- Provides assistance to residents needing additional support during specific activities in a respectful dignified manner.
- Provides one-on-one “well/social” visits to residents who prefer to stay in their apartment or prefer not to join large groups, but enjoy coming out to the common areas.
- Inspires, motivates, and encourages volunteers and fellow team members to engage residents in meaningful, purposeful activities throughout the day and evening.
- Ability to handle multiple priorities.
- Possess written and verbal skills for effective communication.
- Competent in organizational & time management skills.
- Demonstrates good judgment, problem solving and decision-making skills.
- Ability to make responsible choices, decisions and act in a resident's best interest.
- Ability to work semi-independently without direct supervision by following community procedures and guidelines, ability to follow through on assigned tasks.
- Basic proficiency in computer skills, Microsoft Office with the ability to learn new applications.
- Follows all policies and procedures.
- Must be able to relate information concerning residents’ condition.
- Must receive a "not disqualified" criminal background result from Department of Human Services and may not be on the OIG exclusions list.
- Understands and adheres to the Vulnerable Adult and Abuse Policy. Is familiar with the Resident’s Bill of Rights and adheres to it.
Experience
Required- 1 year(s): Experience working with persons who have memory impairment within a Life Enrichment environment.
Education
Preferred- High School or better
