Vital Health is looking for a Childrens Case Manager to provide support in our programs for school-age children. The right candidate will have experience working with children from culturally diverse backgrounds. An NPI and Medicaid provider number is required to create appropriate documentation.
Essential Duties and Responsibilities
- Service children who are currently involved in the youth day treatment program
- Maintain a caseload for youth
- Work with patients in the community
- Help facilitate groups
- Use a strengths-based and trauma-informed approach to support behavior modification and life-skill development
- Facilitate goal setting and treatment planning
- Address barriers that impacts the ability of clients to meet their goals
- Assist with transportation when needed
- Work collaboratively with clients, their families, internal and external team members
- Be willing to work in the community and with community partners
- Provide community resources and referrals.
- Timely document all activities
- Maintain compliance with agency policies and procedures, the Quality Assurance Plan, federal and state regulations, and healthcare industry standards
- Participate in assigned meetings, staff development, and training sessions
Requirements
· 2 or more years case in Case Management
· 2 or more years of behavioral health experience (with youth preferred)
· Familiarity with QMHS documentation preferred
· Active CDCA or CHW certification preferred
· Previous Ohio Medicaid providers are preferred
· Working knowledge of community resources
· Strong interpersonal and engagement skills
· Strong organizational and time-management skills
· Excellent written and verbal communication skills
· Strong computer skills required; experience with documentation in Electronic Health Record
· Ability to support the agency’s mission and philosophy
· Sensitivity to cultural diversity and workplace harmony
· Ability to deliver excellent customer service
Job Type: Full-time
Pay: $18.00 - $23.00 per hour
