Role Description

This is a full-time on-site role for a Youth LSW/LPC at Vital Health in Maumee, OH. The role involves providing support and leadership in youth programs, engaging in youth work, maintaining effective communication, and performing social work duties to empower individuals on their wellness journey.


Qualifications

  • Bachelor's Degree from accredited university or equivalent. (Master's degree preferred)
  • Licensure or certification such as LSW, LISW, LPC, LPCC (and desire to ensure that your license to practice remains active)
  • Knowledge of addictions and mental health complications. Knowledge of community health resources that support life improvement in physical, emotional, mental, and spiritual health, economic security, employment, etc.
  • Knowledge of the stages of alcohol and other drug withdrawals and ability to take appropriate action at each stage.
  • Knowledge of federal, state and municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction including:
  • Confidentiality
  • Patient Rights
  • Professional Code of Ethics
  • Discrimination
  • Ability to be flexible and work as a team.
  • Substance abuse and mental health experience


Responsibilities:

  • Complete diagnostic assessments for new patient admissions. Provide guidance for new patients about orientation procedures and scheduling.
  • Diagnose substance use disorders and/or other co-occurring psychiatric disorders within scope of practice.
  • Work in our children program's during and after school hours.
  • Complete SUD and Mental Health treatment plans and reviews for all patients based on the patient's level of care.
  • Advise and counsel patients on a broad range of personal, family and social problems.
  • Facilitate groups for our Partial hospitalization, IOP and Day Treatment patients.
  • Provide information and referrals to various community resources (for example legal and financial).
  • Make home visits to assess the situation of an individual or family in need as well as to respond to emergencies like a death in the family.
  • Exercise sound professional judgment and seek assistance as necessary to effectively manage patient behavior.
  • Interact appropriately with patients, showing them respect while maintaining boundaries.
  • Remind patients of program rules and regulations as necessary and report infractions appropriately.
  • De-escalate patient concerns and other patient problems.
  • Document all activities in the EHR.
  • Protect the privacy of all patient information in accordance with the organization's privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principles of professionalism as a health care provider.
  • Ensure compliance with regulations and laws regarding patient confidentiality, privacy rights and/or any other regulated subjects
