What are the responsibilities and job description for the Home Health Aide position at VNSW At Home?

1. Provides assistance with personal care which includes:
- Bathing (partial, complete bed, shower, tub)
- Oral Hygiene (teeth, gums, mouth)
- Grooming (hair care, shaving, shampooing, nails)
- Incontinent care
- Toileting, use of commode, bedpan, and urinal
- Feeding
- Dressing
- Assist with walking, use of walker, cane or wheelchair
2. Assists with other functions as assigned and instructed by the nurse or therapists
· Assist with transfers from bed to chair or commode and back
· Ambulation with and without assistive devices
· Range of motion exercise
· Temperature, pulse and respiration
· Measuring intake and output
· Weight
· Assist the clients with self administered medications
· Reinforcing/changing simple dressing
· Maintaining simple records as requested
3. Assists with secondary duties related to the client’s environment
- Client’s personal laundry
- Changes bed linens
- Shopping, if needed
- Light Housekeeping (dusts, vacuums, and straightens the room for client uses)
- Prepares and serves meals according to instructions
- Escort to a physician appointment (only if authorized)
4. Documents care as appropriate and on a timely basis
- Records activities and observations on the “Daily Activity Record’
- Maintains other patient care reports as appropriate
5. Reports significant changes in the patient or his/her care to the nurse as soon as possible.
6. Performs other duties as assigned
Requirements:
- Current certification as a Home Health Aide from a New York State approved training program
- Valid driver's license
- Vehicle available to use for work
- Auto insurance with required company limits
