Voices for a Second Chance (VSC) is a nonprofit organization dedicated to providing essential support and resources to justice-involved individuals as they transition back into the community. Through direct services, advocacy, and programming, we empower returning citizens with the tools necessary to achieve stability, self-sufficiency, and long-term success.

 

Position Summary

The Senior Case Manager will provide comprehensive case management services to individuals reentering society following incarceration. This role involves assessing client needs, developing individualized service plans, coordinating with community resources, and advocating for clients to ensure access to essential services. The Senior Case Manager will also support junior case managers, contribute to program development, and ensure the implementation of best practices in reentry services.

 

Key Responsibilities

  • Case Management & Client Advocacy:
  • Conduct in-depth assessments to determine client needs, strengths, and barriers to successful reentry.
  • Develop and implement individualized case plans, connecting clients to housing, employment, healthcare, substance abuse treatment, and mental health services.
  • Provide crisis intervention, referrals, and advocacy to support client stabilization and reintegration.
  • Maintain detailed case notes and documentation in accordance with organizational policies and grant requirements.

 

  • Program Development & Implementation:
  • Assist in the development and enhancement of case management programs to improve service delivery.
  • Identify gaps in services and collaborate with leadership to create strategies for addressing client needs.
  • Ensure that all program activities align with VSC’s mission and strategic objectives.

 

  • Supervision & Mentorship:
  • Provide guidance and support to junior case managers.
  • Conduct case reviews, offer feedback, and ensure adherence to best practices in reentry case management.
  • Facilitate training and professional development opportunities for the case management team.

 

  • Community Engagement & Partnerships:
  • Establish and maintain partnerships with local service providers, government agencies, and community organizations.
  • Represent VSC at community meetings, task forces, and advocacy groups related to reentry services.
  • Conduct outreach to justice-involved individuals and promote available services.

 

  • Data Collection & Reporting:
  • Track client progress and outcomes to assess program effectiveness.
  • Prepare reports for funders, stakeholders, and organizational leadership as required.
  • Utilize data to identify trends and advocate for policy changes benefiting returning citizens.

 

Qualifications & Skills

  • Bachelor’s degree in social work, criminal justice, psychology, or a related field (Master’s degree preferred).
  • Minimum of 5 years of experience in case management, preferably with justice-involved populations.
  • Demonstrated understanding of the criminal legal system, reentry challenges, and community resources.
  • Strong interpersonal and communication skills, with the ability to build trust and rapport with clients.
  • Experience in crisis intervention, de-escalation, and trauma-informed care practices.
  • Proficiency in case documentation, data collection, and reporting.
  • Ability to work independently while contributing to a team-oriented environment.
  • Commitment to social justice, equity, and the empowerment of returning citizens.

 

Preferred Qualifications:

  • LSW, LCSW, or equivalent professional certification.
  • Experience in grant reporting and program evaluation.
