Job Title: Case Management Specialist
Location: Springfield/Southern IL and Chicago/Chicagoland (remote, but may need to be onsite a few days during training)
Job Type: 5 months contract to hire
Summary:
Responsible for coordinating discharge care, episodic case management, and pre/post-admission counseling for acute patients. Provides education, support, and resources to ensure smooth care transitions and reduce re-admissions.
Key Responsibilities:
- Coordinate discharge planning and case management.
- Collaborate with physicians, discharge planners, and providers.
- Educate members on care plans and medication adherence.
- Assist with appointment scheduling and navigating healthcare systems.
- Refer complex cases to appropriate departments.
- Maintain licensure and comply with HIPAA and corporate policies.
Qualifications:
- RN or LMSW with active, unrestricted license.
- 2 years clinical and 1 year managed care experience.
- Strong communication and customer service skills.
- Valid driver’s license and transportation for occasional travel.
