Case Manager

This case manager role is in the Community Support division. The position is responsible for providing supportive counseling as well as rehabilitative and social work services to persons with mental illness. They are also responsible for coordinating services with auxiliary programs, psychiatrists, other agencies, and the community. The CSP case manager is expected to serve around twenty to thirty clients on their caseload depending on the level of need.

Job Duties (Summary)

  • Providing support services to clients such as outreach visits, supportive counseling, support learning independent living skills, and transportation
  • Helping clients to remain responsible for their own lives
  • Will work closely with housing coordinator and other internal and external partners
  • Provide treatment plans and updates
  • Advocate for clients when they are not capable of doing this for themselves
  • Attend required meetings and trainings, including individual and/or group supervision and other agency committee work when indicated
  • Providing your supervisor and other team members with the information necessary for them to ensure the efficient delivery of services
  • Completing all required paperwork on time
  • Fulfilling other duties appropriate to your position as assigned

Qualifications Masters Degree in related mental health field/social work preferred or Bachelors Degree in related mental health field/social work with a minimum of 1 year of experience. Supervision toward mental health/social work licensure provided.

Compensation and benefits:

This Full-time, exempt position starts at $24.87 and includes comprehensive benefits including paid time off, employer-sponsored health, dental, life, and disability insurances; a 403(b) retirement plan with generous employer match; and educational benefits such as tuition reimbursement and student loan repayment.
