What are the responsibilities and job description for the Individualized Home Support Worker position at WATERBURY ARC?
INDIVIDUALIZED SUPPORT WORKER (22.5 Hours a week) 2:30p to 7pm Monday through Friday
DEPARTMENT: IFS/IHS
QUALIFICATIONS: Minimum of high school diploma, experience working with individuals with intellectual/developmental disabilities preferred. Must have the ability to follow individual plans and document activities. Must possess stellar health and safety practices and have good verbal, written and interpersonal skills that will allow for effective communication amongst program participants, families and staff. The ability to remain organize and work independently is essential as well as completing recommended staff trainings and fulfilling obligations of data collection and tracking. Must be available to evenings. Flexibility in scheduling is a must. Computers skills may also be necessary depending on need. Must have valid driver’s license and may be required to drive personal vehicle therefore, auto insurance is also required.
CREDENTIALS DESIRED: (ANY INTERESTED CANDIDATE MAY BE SUBJECT TO ONE OR ALL OF THE FOLLOWING) DDS Medication Certification, Public Service Licensing, PMT Certification, Defensive Driver Training, First Aid and CPR as well as a general working knowledge of infectious disease and safety precautions. Mandatory agency training such as HIPPA guidelines as well as any other training that may be necessary to provide sufficient support.
PHYSICAL REQUIREMENTS: (DEPENDING ON CASE, CANDIDATE MAY BE SUBJECT TO ONE OR ALL OF THE FOLLOWING: Lifting for reasons that involve personal care or safety. The ability to maneuver wheelchair or medical lift. May be required to accompany individual to gym or participate in physical activity as part of goals or objectives. May be required to clean, lift or manage personal care items, households or living space.

DUTIES INCLUDE BUT ARE NOT LIMITED TO:
- Reading and understanding individual plans and level of need for assigned program participants and adhering to expected outcomes outlined by individual goals and objectives.
- Keeping accurate time sheets and essential documentation in order to prove quality of work, accuracy of service and proof of support.
- Possessing a general knowledge of Medicaid regulations as it pertains to accuracy, billing and fraud.
- Recording pertinent activities on daily, monthly or weekly tracking sheets depending on individual need and submitting said documentation in a timely manner.
- Maintaining a clean driving record as well as public service license as means to protect the health and safety of program participants.
- Attending necessary meetings, in-services and trainings while staying current with all required certifications or endorsements, including but not limited to; medication certification, First Aid and CPR, PMT training, driver’s license, public service license as well as auto insurance.
- Understanding and respecting differences between culture, religion and race and being able to provide service without bias in order to abide by agency mission statement that is to provide the opportunity for all individuals with intellectual/developmental disabilities with a safe environment that allows for personal and communal growth.
- Following assignment guidelines with accuracy and honesty as means to provide suitable supports and maintaining the health, safety and integrity of agency policies and IFS program reputation.
- Utilizing community resources to meet specific goals and objectives defined in program participant’s individual plan.
- Understanding each individual case is different and exuding a willingness to accept this change put forth a solid effort in providing essential services that meet a case by case need.
- Aiding in personal care for program participants including but not limited to toileting, showering, maintaining oral hygiene, changing, dressing, etc.
- Acknowledging any dietary or medical restrictions as well as pertinent information in efforts to avoid harm or injury to program participant. Including but not limited to, medical allergies or food allergies. Food restrictions as well as eating guidelines must be followed closely as well as consistency, texture, and size of food. May be required to assist individual with eating/drinking. Medical restrictions or medical apparatus that may impose hinderance of mobility or obstacles in community involvement or daily activities should be addressed and noted with the goal of alternate planning and emergency protocol.
- Understanding behavioral guidelines and the reason behind the need for individualized support and using that understanding to provide satisfactory service to program participant.
- Protecting dignity and integrity of program participants by respecting their rights as human beings and as essential components in our agency’s mission statement that is to provide individuals with ID/DD with the opportunity to work learn and belong in a safe, friendly and accepting environment with appropriate supervision and care.
