Job Description
Per Diem. Weekends. Responsible for obtaining essential social information from acute care patients and their families, as well as providing counseling services as appropriate. Works in close cooperation with other health care providers, social service agencies, hospital, clinics, and community resources in planning to meet the needs of patients; acts as patient advocate to ensure that the patient is utilizing all services available.
Job Requirements
Requires a Bachelor's Degree in Human Services or related field. Prior experience as a health care system caseworker preferred.

Constantly: (Exists 75% or more of the time) Walking; lifting and carrying 0-5 lbs.
Frequently: (Exists 25-75% of the time) Standing; sitting w/back support; operating machinery; exposure to radiation.
Occasionally: (Exists 5- 25% of the time) Reaching (overhead, forward, lateral, low); climbing; twisting; lifting and carrying 5-25 lbs.; pushing and pulling 5-15 lbs.
Rarely: (Exists under 5% of the time) Stooping (bend at waist); crouching (bend at knees); kneeling; balance; pushing and pulling 15-40 lbs.; exposure to chemical hazards, infect hazards, slippery surfaces.
Sensory Requirements Visual: close paperwork, VDT used; Hearing: conversation, telephone, background noise.
