Youth & Family Support Worker Job Posting OverviewTo provide in-home and community-based support services to children, young adults, and families both individually and as part of a team to support them in achieving their desired level of functioning. Minimum Requirements Minimum Education: High School degreeMinimum Work Experience: At least 5 years experience of direct experience working with children, young adults, and families—specifically those with Serious Emotional Disturbance.Licensure: N/ACertification: N/APerform Supervisory Duties: NoDriver’s license, reliable automobile and clean driving record also required.Job Description/TasksClinical: Provide direct in-home services to children and families referred by DCF, DMH, or insurance. Work as part of a team with the child, parents, and other providers. Have minimum required contact with client(s) according to program specifications. Provide crisis intervention services as needed and as required by position. Provide culturally informed therapeutic interventions to children, young adults, and their families, that promote skill building in the life domains of: social functioning, symptom management, coping skills and activities of daily living.Care Coordination: Collaborate with all members of the team to ensure continuity in treatment, services, and messages delivered. Share resources with the team and with client as learned/needed. Be able to prove transportation for clients to care plan meetings, DMH meetings, and other meetings as applicable. Provide both "soft" services (mentoring, counseling, skill-building) with identified clients as well as "hard" services including transportation, referrals, accessing concrete services such as furniture, food, etc. Engage in regular case consultation and care coordination activities with referral sources and other providers. Provide advocacy and attend meetings with collaterals as requested by the youth and/or family.Administrative: Document all contacts with client and regarding client in client record.
