Salary Range:    $75,000.00 - $85,000.00 Salary/year

Job Description

Responsible for providing direct services as a clinician on the team; providing supervision to each specialty role based on evidence-based practice, trauma-informed care, cultural humility, and person-centered language; and providing administrative oversight to the team.  The Team Leader regularly conducts individual supervision, which may include direction to staff regarding individual cases or side-by-side contact with staff.

 

Essential Accountabilities

  1. Leads daily team organizational and service planning meetings, provides direction to staff regarding individual cases, and regularly conducts individual and group supervision meetings. 
  2. Meets required productivity targets and completes all documentation within assigned time frames.
  3. Monitors and coaches staff on meeting productivity measures.
  4. Directs and coordinates the client admission process and treatment, rehabilitation and support services of the program. 
  5. Participate in staff recruitment, interviewing, hiring, work assignments and orientation and performance supervision.
  6. Develop and implement staff orientation and training.
  7. Develop and maintain program policies and procedures and revise as necessary.
  8. Perform on-call duty for crisis intervention and for staff supervision and consultation.     
  9. Oversee and monitor accurate use of internal electronic health record (Avatar).
  10. Provide guidance in engagement and communication with enrollees.                                                 
  11. Provide leadership through positive modeling and be open to the opinions of all team members.
  12. Provide guidance to ACT staff for identification of potential barriers to care and resolutions to those barriers.
  13. Audit client charts on a regular basis.
  14. Coordinate and monitor services with AOT, mental health and criminal courts, probation, and child protection services on an ongoing basis.
  15. Responsible for crisis intervention requiring 24 hours on call availability.
  16. Provide disciplinary actions to staff in violation of agency, contract or program policy.
  17. Act as a clinical liaison between the team and outside entities.
  18. Document any progress notes in EHR within 48 hours of all client and collateral contacts.
  19. Ensure all documentation is completed in a timely fashion and remain in compliance with agency, OMH, and AOT regulations.
  20. Communicate pertinent consumer and operational information to the Deputy Director, Division Director and Regulatory bodies.
  21. Participate/Organize case conferences and case consultation to ensure a high standard of service provision.
  22. Conduct field based visits. The ACT Program provides 85% of their services in the community.
  23. Participate in OMH approved/required ACT Orientations and Trainings.
  24. Input all data into Vellohealth software application and monitor that all staff are keeping application up to date.
  25. Assists in the planning, developing and implementation of goals and training to further develop participants’ skills, including but not limited to:
    1. Budgeting
    2. Grooming
    3. Nutritional planning
    4. use of public transportation
    5. use of community resources                                                                          
    6. Shopping
    7. Housekeeping
    8. Physical health & development
    9. Adaptive behavior/independent living
    10. Nutrition
    11. Vocational development
    12. Social development
  26. Acts as a role model for participants; furthers participants in researching and obtaining community resources appropriate and applicable to their needs and goals; follows-up on any aftercare/recommendations for treatment. Escorts/ accompanies participant as needed on appointments (medical, psychiatric, substance abuse).

  27. Provide crisis intervention and mental health counseling as needed.

  28.  Participate in client meetings and conduct all 30, 60, 90 day follow-ups on discharged clients.                                              

  29. Required to participate and attend meetings/trainings at various sites that require following different route of travel to worksite.

 

OTHER RESPONSIBILITIES:

  1. Demonstrate ability to: maintain required written documentations; communicate relevant information to other staff and supervisor(s), verbally and in writing.
  2. Ability to work flexible schedules as work schedules, trainings and location are subject to change
  3. Ability to operate an agency vehicle.
  4. Attend all required training’s and any relevant to the position or deemed necessary by supervisor.
  5. Participate in activities of other staff members in their absence or during periods of staff shortage.
  6. Maintain confidentiality at all times.
  7. This does not limit the assignment of duties or exclude the performance of other duties not mentioned as assigned by supervisor, Deputy Director, Director, and/or Senior Management.
  8. Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.

 

Job Qualifications


  • Master degree or higher in social work, psychology, rehabilitation counseling, or a related field, with  licensure/registration LMSW, LCSW, LMHC, OR RN (RNs are encouraged to be masters level or nurse practitioner), or physician (Ph.D, Psy. D, M.D.).  Preference will be given to persons with experience on a multidisciplinary mobile team.   
  • Two years supervisory experience with all levels of staff.  Experienced in meeting regulatory requirements with OMH and other governmental agencies and working with behavioral health population preferred.
  • Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record. 
  • Strong interpersonal and communication skills and knowledge of group dynamics required
  • Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks.

Salary : $75,000 - $85,000
