Program Manager – Adult Foster Care (AFC/GAFC)
Location: Worcester, MA
Reports to: Assistant Director

Position Summary
The Program Manager is responsible for the day-to-day operations, compliance oversight, and staff coordination of the Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs. This role ensures services are delivered in accordance with MassHealth regulations, agency policies, and quality standards while serving as the primary operational lead for assigned programs.
The Program Manager acts as the first point of contact for program staff and works closely with the Assistant Director to ensure smooth operations, staff accountability, and regulatory compliance.
Key ResponsibilitiesProgram Operations
- Oversee daily AFC/GAFC program operations and service delivery
- Monitor caseloads, visit schedules, and service timelines
- Ensure required nurse and case manager visits are completed timely
- Address operational issues promptly to prevent service disruptions
Staff Management & Support
- Supervise program staff including nurses, case managers, and aides
- Serve as first-line support for staff concerns and operational questions
- Coordinate staffing coverage and scheduling needs
- Ensure staff follow agency policies and professional standards
- Escalate unresolved or serious matters to the Assistant Director
Compliance & Documentation
- Ensure compliance with MassHealth AFC/GAFC regulations and program requirements
- Monitor documentation for accuracy, timeliness, and completeness
- Support audit preparation, site visits, and corrective action follow-ups
- Track required forms, physician orders, care plans, and visit logs
Quality & Performance
- Monitor program performance metrics and identify improvement areas
- Support quality improvement initiatives and corrective actions
- Assist with staff training and onboarding related to program operations
Communication & Coordination
- Serve as liaison between staff, leadership, and external partners
- Maintain professional communication with caregivers, members, and providers as needed
- Participate in leadership and program meetings
Qualifications
- Bachelor’s degree in healthcare administration, nursing, social work, or related field (required)
- Clinical license (RN, LPN, or equivalent healthcare background preferred)
- Minimum of 3 years experience in AFC, GAFC, home care, or similar healthcare program
- Strong knowledge of MassHealth AFC/GAFC regulations
- Proven supervisory and organizational skills
- Strong documentation and compliance mindset
- Excellent communication and problem-solving skills
Preferred Experience
- Prior experience managing AFC or GAFC programs
- Experience with audits, site visits, or regulatory reviews
- Familiarity with electronic health record systems
Work Environment
- Combination of office-based work and field coordination
- Occasional travel for program needs or site visits
