Overview:
Key Responsibilities:
- A Home Health Registered Nurse (RN) provides high-quality care to patients in their homes, helping them recover from illness or surgery or manage chronic conditions. The RN collaborates with the healthcare team to develop and implement care plans, ensuring that patients receive the right treatments and interventions.
Key Responsibilities:
- Conduct comprehensive assessments of patients’ health, including physical and psychosocial conditions, and determine home care needs.
- Monitor vital signs and administer medications as prescribed.
- Care Plan Development:
- Develop, implement, and evaluate individualized care plans based on patient needs and goals.
- Coordinate with physicians, therapists, and family members to ensure continuity of care.
- Perform nursing tasks such as wound care, IV therapy, medication administration, and catheter care.
- Educate patients and their families on managing illnesses, injury recovery, medication adherence, and safety precautions at home.
- Accurately document patient care, treatment plans, and progress notes in electronic health records (EHR).
- Communicate effectively with the patient’s healthcare team to provide updates on patient status and progress.
- Active RN license in the state of employment.
- Bachelor’s degree in Nursing (BSN) preferred, or an Associate's Degree in Nursing (ADN) with relevant experience.
- Current CPR certification.
- Experience in home health or community health nursing is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage time effectively.
