ENTITLEMENT SPECIALIST

JOB SUMMARY
The Entitlement Specialist will be responsible for helping clients obtain and maintain mainstream benefits including but not limited to: Social Security including (SSI, SSP, Disability) Medicare including Medicare Advantage), Medicaid, SNAP, Low- Income Home Energy Assistance program (LIHEAP), SCRIE, DRIE and where applicable, assistance with one shot deals for rent arrears. The position is supervised by the Clinical Coordinator.
Please Note: This is a full-time, on-site position. This role will work 3 days at the 129th Street Residence (W,TH, F) and 2 days at the Borinquen Court Residence (M/T).
JOB DUTIES & RESPONSIBILITIES
Responsibilities include but are not limited to:
- Submit new ,or renewal applications with, or on behalf of clients.
- Maintaining up to date information on mainstream benefits eligibility criteria and application requirements.
- Assist residents in obtaining necessary documentation including birth certificates, ID’s, award letters, medical documentation.
- Escort clients to file applications as necessary.
- Monitor benefit application process, resolving problems and advocating for clients as needed.
- Work with client/family/client representative to gather any additional documentation required to finalize benefit applications.
- Work with clients to address any issues that arise, such as inactive Medicaid numbers.
- Provide necessary follow up with respective agencies to resolve any obstacles in the application process.
- Track status of the applications and provide timely and accurate information to Social Workers/Case Managers regarding status of each application and any related problems.
- Submit monthly reports on all relevant metrics in a complete and timely manner.
- Other duties as required.
REQUIRED EDUCATION, EXPERIENCE AND CREDENTIALS
- 4-year degree from an accredited college or university in Social Work or similar discipline or high school diploma or a combination of education and applicable experience working with older adults including persons with mental illness.
- At least 1 year of experience working with older adults, or persons challenged by mental illness and chronic medical conditions.
- Strong knowledge and demonstrated experience with mainstream benefits, eligibility and documentation requirements
- Bilingual Spanish highly preferred
- Ability to speak, read and write English
- Emotionally, mentally and physically able to perform job responsibilities
- Strong computer and organizational skills
- Ability and willingness to travel between facilities as needed.
PHYSICAL REQUIREMENTS
- Prolonged periods sitting at a desk and working on a computer.
- Travel within our sites as needed
