Facilitate and direct the nursing care in the unit/department access, plan, implement and evaluate nursing care. Utilize the nursing process as the means of care delivery.
- Oversees all patient teaching to enable patient to perform home dialysis; coordinates the patient training, supply ordering and home visits.
- Coordinates monthly clinical assessment of each patient and sees that interdisciplinary care conferences are held each month.
- Collaborates with physicians and interdisciplinary team regarding patient problems as they arise.
- Develops the nursing care to achieve optimal patient outcomes in collaboration with the Director.
- Monitors departmental compliance with regulatory standards and guidelines of DOH, DNV, ESRD & DOQI.
- Utilizes patient satisfaction data to improve care and services.
- Provides direct nursing care and on-call coverage for CAPD and hemo in times of need.
Collaborates with the Director in developing and implementing unit specific goals, objectives, values and mission.
- Demonstrates positive leadership within the hospital.
- Promotes teamwork and team building to promote and implement effective nursing standards of care.
- Develops and revises nursing policies and procedures as needed.
- Acts as a change agent to continuously improve nursing services to meet the needs of HAHV patients.
- Acts as a role model and clinical resource to all the nursing staff.
- Works with the medical staff and other departments to solve problems and achieve goals.
- Demonstrates teamwork philosophy through interaction with all HAHV management, employees and physicians
Provides effective communication throughout the department.
- Verbal and written communication is clear and well developed.
- Ensures that lines of communication between staff, management, and physicians are open. Relays appropriate information in a timely fashion.
- Participates in monthly staff meetings to discuss departmental issues and relay information.
- Participates on hospital committees as appropriate.
Assists and encourages staff to reach optimum potential.
- Coordinates and participates (in collaboration with renal education and HAHV education department) CAPD education to nursing staff in department and HAHV.
- Ensures completion of staff training and development programs on a timely basis, including unit based orientation and a competency program.
- Participates in collaboration with renal education, in community pre-dialysis education.
- Provides education for community nursing home staff when needed.
- Participates in collaboration with renal education, in nursing student education in CAPD/Renal
In collaboration with the Director, performance improvement activities are developed for the unit and department.
- Identify PI needs as appropriate, actively contributes to PI data collection and process utilizing the PDCA model.
- Consistently models attitude of continuous improvement to others.
- Documents unusual occurrences appropriately.
- Tracks and analyzes all incidents.
Demonstrates knowledge of information management principles.
- Adheres to all policies and practices regarding confidentiality of electronic communications, including but not limited to: email, telephone, voicemail, fax, internet usage.
- Keeps current on updates to computer system where applicable.
- Uses data and information to assist in decision making.
In collaboration with the Director, the unit budget is prepared – operational and capital.
- Continuously monitors the budget to meet financial goals.
- Effectively uses resources as endorsed by inventory control and pursues more efficient staffing/scheduling, yet assuring appropriate quality care.
- Verifies availability of all supplies required for proper functioning of the department.
Performs any other duties as assigned.

Education:
- Minimum – 2 year college degree; preferred-4 year college degree
Experience:
- Minimum – 2 years acute care experience and minimum of 6 months Peritoneal Dialysis Experience; preferred-2 years dialysis experience
Training/License:
- NYS Licensed and Registered Professional Nurse ; BLS Certification
Other:
- Nephrology nursing certification preferred; med/surg nursing certification preferred
