Western Tidewater CSB is seeking Full -Time ACT Team Leader(s).

Overview:

  • Supervision, coordination, implementation, and maintenance of the Programs for the Assertive Community Treatment (ACT) Team.
  • Completion and review of screenings and diagnostic assessments; determination of medical necessity and consumer’s eligibility for services.
  • Provision of clinical supervision and maintaining documentation in accordance with state/federal regulations and agency policy

Additional Key Responsibilities:

Major Duties and Responsibilities:

  • Provide direct supervision to ACT staff, including provision of clinical and procedural guidance, monitors the provision of services, utilization, revenue and disbursements, and adherence to agency policy. Oversee staff scheduling to ensure sufficient resources are in place to meet program or department demands. Conduct staff meetings in accordance with regulations. Participate in meetings with the Crisis Response Center and other agency personnel as indicated to coordinate cases to enhance the provision of services.
  • Develops and maintain protocols or admission policies regarding the admission and referral of individuals.
  • Monitors clinical documentation to ensure quality documentation standards. Participates in statewide ACT and CSB training conferences. Maintains relationship with community partners.
  • Produce client-related service records and maintain accurate and up-to-date documentation consistent with agency policy and procedures.

Knowledge and Skills:

  • Medicaid and Licensure regulation guiding ACT services, consumer rights and the cause of MH/MR/SA conditions including clinical and developmental issues
  • Services that are available in the community including primary health care, support services, eligibility criteria and intake process and generic community resources.
  • Different types of assessments, including functional assessments and their uses. Treatment modalities and intervention techniques. The service planning process and major components of the consumer service plan and ISP. The use of medications in the care or treatment of the population served
  • All applicable federal and state laws, regulations and local ordinances. Human Rights Regulation/Reporting procedures and confidentiality rules and regulations.
  • Identify and document an individual’s need for resources, services and other supports. Identify and document how resources, services and natural support can be utilized to promote achievement of a consumer’s personal habilitative/rehabilitation and life goals. Use information from assessments, evaluations, observations and interviews to develop the ISP.
  • Coordinate provision of services by diverse public and private providers.

Qualifications:

  • Master’s Degree in Psychology, Social Work, Counseling or related Human Services field required.
  • Licensed or eligible to work toward licensure (LPC or LCSW) upon approval by the respective Board required.
  • A minimum of five (5) years’ work experience in a human services field, preferably in a mental health setting.
  • Valid current driver’s license and acceptable DMV record at time of hire and continuously thereafter
