Immediate Eligibility for Holiday Pay
Education & Experience:
- License Practical Nurse (LPN) in the state of Ohio
- Associate or Bachelor of Science Degree in Nursing or in related field
- Must be CPR/First Aid Certified
- Ohio driver’s license, current auto insurance and access to a car during working hours.
- Home Health experience with terminally ill patients preferred.

- Strong verbal and written communication skills.
- Demonstrate flexibility, excellent assessment skills, and ability to supervise others and the confidence to work independently.
- Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.
- Active listening skills with the goal of understanding
- Possess skills of working with team having positive outcomes
Job Duties:
- Assume responsibility for patients and families under the care of Zusman Community Hospice. May include assessment, planning, implementation and evaluation phases of the nursing process.
- Initiate communication with attending physicians, other hospice team members and agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain up-to-date records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
- Perform other related functions as required by the Director.
