We are a dynamic and compassionate skilled nursing facility dedicated to providing high-quality care to our residents. We are seeking an experienced MDS (Minimum Data Set) Nurse to join our interdisciplinary team, ensuring optimal clinical outcomes and accurate resident assessments.
Position Summary:
The MDS Nurse is responsible for coordinating the comprehensive assessment of each resident to meet regulatory requirements and to ensure the delivery of quality care. The role involves completing and overseeing MDS assessments, care planning, and managing the resident assessment instrument (RAI) process to support resident care plans and reimbursement.
Duties
Complete, review, and submit MDS assessments within regulatory timeframes.
Collaborate with interdisciplinary teams to ensure the accurate development of individualized care plans based on assessment data.
Monitor residents’ health status and update MDS documentation accordingly.
Ensure compliance with federal and state regulations, including the RAI process and Medicare/Medicaid requirements.
Educate staff on MDS processes, best practices, and compliance requirements.
Actively participate in care planning meetings, ensuring resident needs are identified and met.
Assist with quality assurance and performance improvement initiatives related to resident care.
Communicate with residents, families, and staff to ensure continuity of care.
Requirements
Current RN or LPN license in OHIO.
Minimum of 2 years of experience as an MDS Nurse in a skilled nursing facility.
In-depth knowledge of MDS 3.0, care planning, and reimbursement regulations (Medicare/Medicaid).
Strong organizational, analytical, and communication skills.
Proficiency in electronic medical records (EMR) systems, with experience in MDS software preferred.
Detail-oriented with a focus on compliance and quality care outcomes.
