NP - House Call Program

Nation-wide primary care organization geared towards the geriatric population seeking Nurse Practitioners to join them as they build out a home care program in Philadelphia. This is a full time permanent position with a 4-6 week orientation/training period, extensive PTO, large CME stipend, malpractice coverage and 401k with match available from day one.

Job Function

• Work on a care team with a scribe, nurse and social worker - leadership prepares schedule and creates zones for the day.
• Provide primary care to patients in their homes or option to be hired as a transition care NP who specifically sees patients in their homes immediately following a hospital visit.
• Follow a value-based care model within this large managed care organization.

Job Requirements

• Board Certification Required/
• PA State License Required.
• Training as FNP, ANP.
• Previous home care, nursing home or primary care experience required (1 yr minimum).
