Responsibilities and requirements for MDS Nurse Coordinator
- Conduct and coordinate the development and completion of the resident assessment (MDS)
- Maintain and periodically update written policies and procedures that implement MDS and care plan
- Assist the resident in completing the care plan portion of the resident’s discharge plan
- Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents
- Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings
- Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents
- Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews
Qualifications
- Valid RN License in the state of VA
- Must have at least 1 year of MDS coordinator & long-term care setting experience preferred; however willing to train the right candidate.
- Clinical experience, education or specialty skills specific to geriatrics is desired
- Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long term care.
- Great interpersonal skills for effective daily interaction with Residents, family members and all members of the Interdisciplinary Team
- Must be a self-starter and a high attention to accuracy and details and possess good time management skills
- Ability to make independent decisions and follow through with limited supervision.
- Full vaccination against COVID-19 or a medical/religious exemption is required.
