We are looking for a Home Health Registered Nurse to join our team PRN and help our patients remain comfortable, at home.

You Will:
- Be responsible for overall planning, providing and coordinating quality care to achieve positive patient outcomes in collaboration with the physician.
- Perform patient assessments (e.g. OASIS) in order to implement the best Plan of Care.
- Supervise the care being provided to the patient by Licensed Practical Nurses and paraprofessionals (Home Health Aides) through regular home visits.
- Communicate with family members and other members of the patient's multi-disciplinary team.
What We Need from You:
- A commitment to helping our home health patients remain comfortable, at home.
- Proof of graduating from an accredited school of professional nursing, BSN preferred.
- Current RN license from the State of Ohio.
- Previous experience in home health care setting preferred.
- Superior organizational and time management skills.
- Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories.
- Current CPR certification.
- Demonstrated compassion, empathy and strong customer service skills.
- A completed and successful TB test, physical examination and a criminal background check.
