Job Summary
Responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of healthcare services.
Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.
Participate in an on-call rotation to ensure patient and customer needs are met in a timely fashion.
Performs other duties as assigned.
Minimum Education/License/Certification Requirements
- Bachelor's or Masters Degree in Social Work, Master's Preferred, with intention of obtaining LSW or LCSW.
