Care Coordinator

This position will manage a caseload and work closely with the clients behavioral and physical health care team to provide direction for treatment services. Meeting directly with clients and their families to assist in navigating their mental health wellbeing.
Willmar Office Location
Full Time (Monday- Thursday 7:30 am -5:00 pm) (Fridays 8:00 am - 12:00 pm)
Job Duties:
- Answer calls and emails in a warm, kind, and prompt manner. Direct communications as appropriate with the client's care team.
- Facilitate referrals both internally and services outside the mental health specialty care and monitor outcomes.
- Assisting with scheduling and confirming appointments and transportation needs and or resources .
- Assisting new clients accessing services at Woodland Centers in person, and or over the phone.
- Monitoring admissions and discharges from hospitals and inpatient units and providing follow up care.
Requirements:
- High School Diploma or equivalent.
- Two years experience working in a behavioral health clinic, human services, or similar setting providing customer service.
- Maintain a valid drivers license, proof of insurance, and a clean driving record.
- Ability to multitask, pay close attention to details, work in a fast-paced environment and work independently at times.
- Experience in technology with electronic health records and data entry.
- Bilingual in Spanish is preferred.
