Woodland Manor, a skilled nursing facility, is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team on a PRN or part-time basis. In this role, you will assess, develop, and implement therapy programs to help residents regain mobility, strength, and independence.
Responsibilities :
- Provide high-quality physical therapy care within the PT scope of practice, following established standards and procedures.
- Assist in the evaluation process and develop individualized treatment plans, including short-term and long-term goals.
- Collaborate with the rehabilitation team to ensure optimal patient outcomes.
- Educate patients, families, and caregivers on therapeutic exercises and techniques to support recovery.
