We are seeking a dedicated and skilled Nurse Practitioner with Wound Care experience to join our clinical team. We are a mobile advanced wound care clinic, and are looking for an NP with a strong wound care background who will treat patients with chronic non-healing wounds in their homes. We combine advanced clinical protocol, the newest wound care technology, and home-based visits in both urban and rural areas to ensure all patients get the care they need.
This role requires a high level of autonomy, clinical expertise, and the ability to travel to patient locations. Starting Part-Time, with ability to become Full-Time.
The mobile wound care NP will be responsible for assessing, diagnosing, treating chronic wounds. The NP will provide direct patient care, collaborate with office staff and a collaborating physician, as well as develop individualized treatment plans to promote healing and prevent complications. This role requires a high level of autonomy, clinical expertise, and the ability to travel by car to patient locations. We will discuss and arrange a maximum distance that you are willing to travel from their home, based on your desired distance limitation. Patient visits will be conducted on a once-per-week basis per patient, typically on the same day each week.
Essential Job Functions
- Conduct comprehensive assessments of patients with chronic non-healing wounds.
- Develop individualized treatment plans based on patient needs and evidence-based practices.
- Perform wound care procedures, including dressing changes, debridement, and the application of advanced wound therapies.
- Collaborate with Home Health agencies and caregivers, provide them instructions and coordinate schedules.
- Collaborate with other care providers involved with that patient such as Primary Care, specialists, etc.
- Educate patients and their families on wound care management, prevention strategies, and lifestyle modifications.
- Document patient progress accurately with photographs and measurements and meticulously record in details within electronic health records system (EHR).
- Participate in quality improvement initiatives related to wound care services.
- Stay updated on the latest advancements in wound care technologies and treatments.
- Order and interpret diagnostic tests such as blood tests, X-rays, etc.
- Prescribe medications, dressings, and treatment regimens within the scope of practice.
- Order and interpret diagnostic tests related to wound care management.
- Adhere to all laws and regulations including HIPAA, as well as company policies and protocols
Job Type: Part-time
Pay: $80.00 - $120.00 per hour
Expected hours: 10 – 30 per week
