Schedule: Variable between 7:30 AM to 7 PM MAXIMUM 42.5 hrs/Week ( Including Lunch)

Minimum Qualification: Education, Experience, Licensure:
For Nurse Practitioner: A Master’s Degree in Nursing with certification as an advanced practice registered nurse is required. Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia, requires national Certification by one of the recognized APRN Certification Centers
For Physician Assistant: Bachelor’s Degree as a Medical/Surgical (as applicable) Physician Assistant is the minimum degree required. Masters level education as Medical/Surgical (as applicable) Physician Assistant preferred. A graduate of an American Medical Association (AMA) approved Physician Assistant (PA) training program required. Certification by the National Commission for Certification of Physician Assistants is required. Must be licensed or eligible for licensure by the WV Board of Medicine.
Summary: The Advance Practice Professional (APP) is an individual with expertise in the management of the medical patients throughout their lifespan, with a focus on both acute and chronic condition management for the patient to maintain optimum health. The APP completes comprehensive patient health evaluations (including biological, social, and psychological assessments) with the appropriate clinical/faculty team. The APP assesses and coordinates the educational and psychosocial needs of the patient and caretakers with a focus on acute issues, maximizing the stability of chronic conditions, disease prevention, and overall medical management of the patient. The APP develops and reviews the plan of care with the other members of the medicine clinical team, participates in directing that care, and coordinates planning for patient follow-up across the continuum of care (as applicable).

Manages comprehensive and individualized care for patients and their families in the clinic. Completes pre-procedure evaluations. Performs, interprets, and communicates results of diagnostic tests in the outpatient setting. Consult and communicate with clinic staff regarding abnormal findings in the evaluations of patients. Performs history and physical examinations and documents findings in the medical record. Facilitates patient follow-up maintenance by coordinating appropriate services and tests prior to scheduled outpatient follow-up appointments in the clinic (as applicable). Performs necessary procedures following established clinical guidelines and Protocols and scope of practice (SOP).
Manages comprehensive and individualized care to patients, taking into account their co-morbid conditions and overall disposition. Develops a comprehensive individualized plan of care for patients, which includes teaching, follow-up, and necessary referral to other members of the health care team as necessary. Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification. Identifies unstable or complex patient care situations and facilitates appropriate intervention in the clinic in collaboration with the medicine physician/patient care team. Evaluates patients in the outpatient setting. Acts as a coordinator for follow-up procedures and tests in the outpatient setting (as applicable).While in the clinic setting, assist in collecting outcome measures (as applicable). Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation, and management of the individual needs of patients/families. Demonstrates the ability to utilize essential equipment in a safe manner. Recognizes emergency situations and initiates effective emergency care. Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings.
Promotes an environment that facilitates learning by responding to appropriate Patient/family preferences, priorities and readiness to learn. Updates own knowledge base throughout the year. Completes all identified clinical competencies. Serves as a resource person for staff caring for the patient. Provides educational programs to professional and paraprofessional personnel in area of expertise. Mentors office personnel in all levels of clinical practice.
4. Communicates effectively with patients/families, physicians and ancillary services at all levels to assure coordinated efficient patient care. 4.1 Documents the plan of care, interventions and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards 4.2 Documents all interventions in the patient’s medical record following approved WV Primary Care policies and compliance standards for documentation.
5. Collaborates with other leaders and staff to effectively achieve needed change. 5.1 Provides leadership in the development and implementation of changes in practice that positively impact patient outcomes. 5.2 Works collaboratively with medical and office staff to facilitate creative problem solving and provide professional support. 5.3 Participates and provides leadership for committees and task forces. 5.4 Documents hours and submits billing reports, as appropriate.
6. Participates in the ACO.
6.1 Applies information from Performance Improvement activities, changing practice as necessary, to improve care provided to patients. 6.2 Reviews and interprets wellness data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary. 6.3 Assists practice Physicians, Administrators, and Directors in the collection of outcome measures.
