This full-time role is essential for organizing and coordinating medical services and follow-up care for CHCY patients.

Review complete class specification for Care Coordinator here.- Works cooperatively with patients and practitioners to identify problems/barriers, appropriate actions/steps, and timelines for completing health care goals.
- Follows-up on no-show appointments of specifically identified patients and expedites and facilitates patient referrals to specialty care.
- Provides patient education as to the importance of following through on prescribed treatments. Works with community providers to ensure that referred diagnostic testing is scheduled and completed timely.
- Provides patient education regarding CHCY services and specific program eligibility.
- Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the patients receiving care coordination.
- Assists with coordinating local marketing initiatives including advertising, materials development, and distributions such as flyers and patient handouts. Helps plan and implement community events such as health fairs, group presentations, and professional education activities.
- Performs other job-related duties as assigned.
Education:
High school diploma or equivalent.

Must possess a valid Arizona driver’s license.
